  • At The Elms Center, our personalized short-term rehabilitation program helps you regain strength, independence, and confidence after a hospital stay, illness, or injury. Our dedicated team of nurses, therapists, and healthcare professionals provides individualized care focused on recovery, reducing rehospitalization, and helping you return home safely and successfully.

  • At The Elms Center, our long-term care program provides compassionate 24-hour skilled nursing care in a warm, supportive environment. We focus on each resident’s unique needs, promoting comfort, dignity, independence, and quality of life through personalized care, engaging activities, and meaningful connections every day.

  • At The Elms Center, our rehabilitation team provides personalized physical, occupational, and speech therapy to help you recover strength, mobility, and independence after illness, injury, surgery, or hospitalization. Working closely with our skilled nursing staff, we create individualized care plans focused on achieving your goals and helping you return home safely and confidently.

  • At The Elms Center, we provide compassionate memory support for residents living with Alzheimer’s disease and other forms of dementia. While we do not have a secured memory care unit, our experienced staff are specially trained to support individuals with cognitive challenges, promoting comfort, dignity, meaningful engagement, and a familiar daily routine in a safe and supportive environment.

The Elms Center is a skilled nursing facility offering post-hospital, short-term rehabilitation, and long-term and respite care.  We accept Medicare, Medicaid, and most private insurance. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. The Elms Center is an AHCA Bronze Quality Award Winner and a We Honor Veterans community partner!
