Long Term Care at The Elms provides 24-hour skilled nursing services for elderly residents and people with a chronic condition or a prolonged illness. Our staff is devoted to the creation of a comforting environment and focused on helping each person achieve their highest level of independence and vitality.

We offer numerous care options that combine rehabilitation therapy and skilled nursing care but are tailored to your unique needs. At the foundation of each of our service offerings is Rehabilitation Therapy which utilizes occupational, physical and speech therapy as part of the overall care plan to get you home as quickly as possible. Specialized training enables our therapists to provide care to patients with complex diagnoses, helping them to overcome deficits that result from illness, disease, disability, injury, surgery or the aging process.
The Elms Center is a skilled nursing facility offering post-hospital, short-term rehabilitation, and long-term and respite care. We accept Medicare, Medicaid, and most private insurance. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. The Elms Center is an AHCA Bronze Quality Award Winner and a We Honor Veterans community partner!

My mother in law was at The Elms. It was her 3rd facility that she was in and by far the best. The staff and atmosphere is so welcoming. Mckenzie and all the folks communicate so well and they treat your family member like they are their own. My mother in law loved being there. Would recommend them highly.

The folks at The Elms are so caring and compassionate, I couldn’t imagine a better place for my Mom to be! They treat her like their own mother, and she loves it.
